Along with the standard WCCUSD Emergency Contact form, parents are required to complete a Disaster Preparedness Form (DP Form) for each child in its entirety. This years DP Form was shortened considerably to reduce redundancy between the two. Please note, if you do not have long-distance contacts outside the Bay Area or California, please check the box on the right hand side of the form so we know this information was purposefully omitted. A paper copy of the form will be sent home during the first week of school. Alternatively, you may use the fillable PDF file and print your own copy to return to school. Please return the DP Form in your child’s Blue Folder or place it in the PTA Disaster Preparedness inbox on the tall black shelves in the school’s main office.
